A transfusão sanguínea é uma prática amplamente empregada ao redor do mundo e, mesmo sendo fundamental na medicina atual, ela ainda apresenta riscos aos pacientes transfundidos, mais conhecidos como efeitos adversos. Esses efeitos adversos possuem como uma das causas os leucócitos presentes nos hemocomponentes transfundidos. Sendo assim uma das formas de se reduzir a gravidade e quantidade dessas reações transfusionais indesejadas é através da leucorredução de hemocomponentes. Também chamada de desleucotização, essa prática consiste em remover a maior parte dos leucócitos presentes no hemocomponente que será transfundido. Através de uma revisão narrativa de artigos pertinentes e relevantes à área, dando-se preferência a artigos mais recentes, porém incluindo textos mais antigos que sejam esclarecedores, foram expostos os métodos mais utilizados de leucorredução, como a utilização de filtros leucorredutores e a coleta de hemocomponente por aférese, as vantagens que essa prática apresenta, focando na diminuição dos efeitos adversos relacionados, e as desvantagens, associadas principalmente aos maiores gastos relacionados realização de leucorredução em grande escala e a legislação pertinente à área. Resumo
The blood transfusion is a practice widely used around the world and, even though it is fundamental in current medicine, it still presents risks to the transfused patients, known as the adverse effects. These adverse effects are also caused by leukocytes present in blood components, therefore one way to reduce the severity and quantity of these unwanted transfusion reactions is by the leukoreduction of blood components. Also called deleucocytation, this practice consists of removing most of the leukocytes in the blood component that are going to be transfused. Through a narrative review of articles relevant and pertinent to the area, giving preference to more recent articles, but including older texts that are clarifying, the most used methods of leukoreduction were exposed, such as the use of leucoductor filters and the collection of blood component by apheresis, the advantages that this practice presents, focusing on the reduction of related adverse effects, and the disadvantages, mainly associated with higher expenses related to large-scale leukoreduction and the relevant legislation.
